Beschrijving:
The Caravan departed Itaituba (ITB) at 06:40 on a flight to Jacareacanga (JCR). Jacareacanga Radio was contacted five minutes later. The crew were informed that visibility was poor: ceiling at 0 feet with a visibility of 300 meters, which was below minimums. The Caravan circled the airport to wait for the weather to improve. At 07:37 Jacareacanga Radio tried to contact the flight, but no answer was received. It appeared that the airplane had flown into the ground.
Investigation revealed that the pilot was underqualified. The flight was being operated as an air taxi flight, which meant that it had to be piloted by an ATPL pilot. The pilot however just had a PPL and did not have an IFR rating.

(1). Condições Meteorológicas Adversas - Contribuíram
As condições meteorológicas eram desfavoráveis na localidade de destino para a realização do vôo visual. Havia uma camada de nevoeiro próxima a floresta .

(2). Deficiente Instrução - Contribuiu
O Comandante não tinha qualificação para voar aquele tipo de aeronave e não possuía habilitação para o vôo por instrumentos.

(3). Deficiente Julgamento - Contribuiu
Os pilotos prosseguiram no vôo, mesmo sabendo das condições meteorológicas adversas na localidade de destino.

(4). Indisciplina de Vôo - Contribuiu
Um dos pilotos não tinha a qualificação para voar aquela aeronave e, além disso, houve desobediência intencional às normas e regras de tráfego aéreo, quanto a não observância do nível mínimo de vôo IFR no setor.

(5) Deficiente Supervisão - Contribuiu
Houve deficiente supervisão da empresa responsável ao deixar que um piloto não qualificado viesse a operar a aeronave.

(6) Deficiente Coordenação de Cabine - Contribuiu
O vôo IFR sem obedecer às regras ditadas na IMA 100-12, conduzido abaixo dos limites e fora do perfil da descida são indícios de que não havia um planejamento/coordenação nas operações realizadas na cabine de comando.
