"THE AIRCRAFT CRASHED INTO A RIVER WHILE ON FINAL APPROACH TO LAND. THE PILOT REPORTED THAT HE ALLOWED ENGINE RPM TO DROP, AND AT THE SAME TIME APPLIED COLLECTIVE PITCH. THE AIRCRAFT STRUCK THE WATER IN A STEEP DESCENT. THERE WAS NO REPORT OF ANY FAILURES OR MALFUNCTIONS.
The National Transportation Safety Board determines the probable cause(s) of this accident as follows:

ROTORCRAFT FLIGHT CONTROLS, IMPROPER USE OF: PILOT IN COMMAND
Contributing Factors:

Despite initial assertions that the aircraft was a write off (damaged beyond repair), and the fact that the registration N90218 was cancelled on March 22, 1989, this particular Robinson R22 was rebuilt, and registered in the UK on March 28, 1989 as G-BPTR. However, as G-BPTR, it was written off for the second and final time on August 23, 1992 when it struck overhead power cables and crashed at Lower Stretton, near Warrington, Cheshire (see link #6)
