de Havilland DH.82A Tiger Moth "122" of Aeronaútica Militar: Written off (destroyed) 24 April 1939 when crashed at Viseu, Centro Region. Aircraft caught fire on impact with the ground, and was burnt out. Both persons on board were killed. According to a rough translation into English of the original Portugese blog (see link #3 for ther original Portugese text):

"April 24, 1939
In an accident in Viseu, the Tiger Moth 122 of the Military Aeronautics is destroyed, the pilot Sergeant Abílio Alberto Maia and the civilian Henrique Mourato Vermelho dying. This plane was abused by the injured pilot after he made a repair of the airplane, without the legitimate pilot having authorized the flight".
