Narrative:
A Bell 412EP helicopter, operated by Guardian Helicopters Inc. was conducting a fire fighting activity in the vicinity of Limache, Valparaiso Province, Chile with 2 crew members on board. The helicopter was performing water pick up with a bambi bucket in the Lliulliu reservoir, which is at 800 feet above sea level, when it crashed in the water. There were no injuries and the helicopter was substantially damage. According to premiminary information received, the weather was not a factor and there was no mechanical problems on the aircraft.

CAUSE
Loss of directional control of the helicopter when the tail rotor impacted the water, causing the pilot in command to capsize the aircraft in the water.
CONTRIBUTING FACTORS
1. Shallow water depth of the reservoir, which allowed mud to enter the interior of the Bambi Bucket, clogging it.
2. Failure to maintain a separation distance between the helicopter and the water when moving the aircraft backwards.
3. Performing a manoeuvre by the accompanying commercial pilot without having the required skills for this type of operation.
